DCA Printable Forms

 If you do not have access to a printer, we can send them to you.

After printing out the needed forms please fill them out completely and mail all of them to: DCA Please do not fax enrollment forms!

New Students:

Application - Print two per student.

Enrollment - Print two per student.

Student Record Release - Print two per student.

DCA Release - Print two per student.

* Note - Fill out 2 but only send in 1

Returning Students:
